I noticed nobody else has jumped in and attempted to answer your questions. Your "requirements" suggest to me that money is no object. If that's the case, you
might find something that meets your needs. I should add here that I am neither an expert nor rich.
I use
TelkomSpeedy which is relatively cheap. It's ADSL with speeds "up to 1Mb/s" for download and "up to 128Kb/s" for upload. Not exactly blazing stuff, but when it works, it's passable (just). Especially for the price. All you need is a telephone line and an ADSL modem. Their "Unlimited" package costs Rp750,000/month (plus 10% tax). So, it's not the "best speed" and "reliability" is a term in Indonesia that often leaves a lot to be desired. The "unlimited" part seems OK. I haven't noticed any speed throttling and stuff like that, so far.
Last month, I successfully downloaded 14GBs of music and movies, without any significant pain. Made quite a few Skype video calls. The only major problem was electricity black-outs in my area.
There is no shortage of companies here that claim much better speeds/reliability. They all cost a lot more, for initial equipment, set-up, and monthly fees. This is not that different from companies in some other countries where you can be lead to believe that you can get astonishing broadband speeds in the middle of a jungle. Maybe you can. For a price.
I have looked at various internet satellite providers. Some claim they can provide you with great connections no matter which country you live in, or where. You just have to buy a bunch of expensive gear to do that, and pay the corresponding charges for the privilege. A quick Google search should put you in touch with them.
There are many internet service providers in Bali claiming they provide premium broadband experiences. For example:
Blueline (1MB/s up and down),
GlobalXtreme ("up to 512Kb/s" for Rp1,100,000/month after their Rp1,500,000 set-up fee),
TelkomselFLASH (don't even bother unless they get their act together one of these days),
IndoSAT,
BaliORANGE, etc.
There is another one, an Australian company which offers "Super Fast Internet", 24hr unlimited connection, anywhere in Indonesia. I rang them once to find out more. It was too expensive for me. If you're interested, you could have a look at their website:

I don't think so, at least not yet. You can buy a satellite dish and tune in to hundreds of overseas' TV channels for free. And
TelkomVision now offers pre-paid monthly vouchers so you can tune in to movie channels, (like StarMovies, Cinemax, HBO, etc). The vouchers start from Rp30,000/month.
I hope this helps you a little bit. Others might have better suggestions or ideas.
