I have a pleasure boat captains certificate but not for a ship that size, so no real experience. However, I don't think that a roll-on-off (RO-RO) ferry just over 21 years old was that ancient. I'm sure that lots of RO-RO in Seattle/Vancouver are at least that age.
Also, I wouldn't suspect that a ship made in Japan would have inferior quality steel.
The weather was calm so I suspect the investigators are looking at something submerged, and not on the charts, holing the hull.
edit: I'm not eliminating gross negligence but we'll wait and see what investigators turn up before I travel to Banyuwangi by ferry.....anytime soon.
