Heads up, I arrived here 3 days ago and the visa on arrival fees have changed since a month ago. People paying in AU$ or IDR used to pay more than the US$ amount when converted but they are all the same now.
New fees:
US$25 (have always been this amount)
AU$25 (used to be about $28 even though the AU has been above par for awhile)
IDR 250,000 (used to be a bit more from memory)
So if you are an Aussie coming here don't worry about trying to save a few bucks from bringing US$25 for your visa fee.
PS: I have also been getting the best x-rates.. ysterday I got 10,280 IDR for AU$1

I don't have a good memory of bank branches exchange rates, mostly much worse than the official exchanges offices.
Go to Jl. Raya Kerobokan, you find there a BMC, a MoneyExchange and another (forgot the name) all nearby. From my experience BMC rate is always bit lower than the others, so go to one of those. Easy to compare as you find them on 100m street length.
They are all three trustful and have good service, I go there all the time. It's worth to check the rate on the billboard outside and ask the rate before exchanging as sometimes they forgot to change the billboard (happened twice and after I told them, they still gave me the best rate of both).
Try not to exchange during weekends or near official holiday (as now with Nyepi) as rates are down because of that. ;-)

i have found most of the circle k money changers (or ones next to) have good rates. about 200-500 lower than the forex rate.
