I haven't even got it yet, but probably will next year. It'll be compulsory for everyone in 2019.
I can get it because I'm married to an Indonesian and am a permanent resident (KITAP) although I only know this from lurking on the Expat Indo site. Technically, in the written law, the only mention of foreigners getting the BPJS are foreigners with a working visa (having worked longer than 6 months). They MUST sign up and I've read that a few foreigners on the retirement visa slipped through the cracks and signed up when the system was new but are now refused.
It makes sense that retirees can't get it as they're supposed to be adding to the wealth of the country, not subtracting, and are therefore required to have their own insurance.
